A medical answering service that books the visit, not a message
A medical answering service picks up when your front desk cannot. Most still put a human operator on the line who writes a message for staff to return in the morning. Clinic Answering Service answers every call with HIPAA-compliant AI, books the appointment in your EHR, checks the patient's insurance and texts the confirmation, for US medical, dental, physical therapy and behavioral health practices.
What is a medical answering service?
A medical answering service is an outside team or system that answers a practice's phone calls during the hours staff are unavailable: nights, weekends, lunch and the busy morning block when every line is lit. Its job is to make sure no patient reaches voicemail, that urgent calls reach the on-call provider and that routine requests are handled before the patient calls the practice down the road.
There are three kinds today. Human answering services such as Ruby, and hybrid services such as Smith.ai, use live agents. General AI receptionists such as Goodcall and Rosie answer with AI but are built for salons, contractors and law firms. Clinic-specific AI answering services such as Clinic Answering Service are built around the EHR schedule, insurance eligibility and HIPAA.
Human vs AI medical answering services compared
The difference is what happens after "hello". A message has to be returned; a booking is already on the schedule.
| Question | Human answering service (e.g. Ruby, Smith.ai) | General AI receptionist (e.g. Goodcall, Rosie) | Clinic Answering Service |
|---|---|---|---|
| Who answers | Live agents, sometimes with AI first | AI voice agent | AI voice agent; your staff by warm transfer |
| What happens to a booking request | Message, or booking in a shared calendar | Booking in a calendar or CRM | Real slot in the built-in schedule; EHR write-back (OpenEMR first) sandbox today, planned |
| Insurance check on the call | No | No | Yes, X12 270/271 (sandbox until a clearinghouse is contracted) |
| HIPAA and a signed BAA | Varies: Smith.ai's medical page said it is not HIPAA compliant when we checked; ask Ruby | Varies: Goodcall advertises "HIPAA Ready"; we could not find a BAA offer from Rosie | BAA on every plan, including Solo |
| Pricing model | Per call or per minute bundles | Per agent or per minute | Flat monthly: $149, $349 or $699 |
| Hold time on a busy morning | Depends on agent staffing | None | None; every line answered at once |
| Best for | Businesses that want a human voice on every call | Non-clinical small businesses | US practices that want calls turned into booked, covered visits |

How much does a medical answering service cost?
Human services charge for each call or minute an agent spends, so the bill grows with your volume. Smith.ai, for example, publishes $300 for 30 calls and $2,100 for 300 calls a month, with add-ons for booking, Spanish and recordings. That is $7 to $10 per call.
Clinic Answering Service charges a flat monthly fee for connected talk time. 300 calls at about three minutes each is roughly 900 minutes, which fits inside the 1,000-minute Practice plan at $349 a month, including EHR write-back, live eligibility, Spanish and recordings. Spam and wrong numbers never count.

300 calls a month, side by side
| Per-call human service (Smith.ai Pro tier) | $2,100 |
| Clinic Answering Service Practice (1,000 min) | $349 |
| Clinic Answering Service Solo (300 min, short calls only) | $149 + overage |
Assumes about three minutes per call. Extra minutes are $0.35. Full plan comparison.
When is a human answering service the better choice?
Be honest about your calls. If most of them are long, emotional conversations, such as hospice family calls or complex billing disputes, a trained human agent is worth the per-minute cost. If you want a human voice on literally every call, AI is not for you.
For most clinics, though, the bulk of calls are bookings, reschedules, cancellations, insurance questions, directions and refill requests. Those are exactly the calls where an AI clinic answering service is faster than a person: it picks up on the first ring, sees the real schedule, checks coverage in seconds and never puts anyone on hold. The calls that need a person still reach one, with a whisper summary, so your staff spend their time on the conversations that matter.
Is a medical answering service HIPAA compliant?
Only if it signs a Business Associate Agreement and keeps the safeguards in it. Any answering service that hears a patient's name and reason for calling handles protected health information, so it is a business associate under HIPAA. When we checked in October 2026, Smith.ai's medical page stated that it is not HIPAA compliant; confirm with the vendor. Clinic Answering Service signs a BAA on every plan, binds its sub-processors to BAAs, encrypts with AES-256 and TLS 1.2+, hosts production PHI in an AWS US region, reports breaches within 10 business days and never uses patient data to train AI models. Our 12-point HIPAA checklist lists the questions to ask any vendor, and the security page shows how we answer them.
What a medical answering service should do on every call
- Answer instantly, 24/7, in English and Spanish.
- Confirm identity with two identifiers before reading back anything.
- Book, move or cancel in the real schedule, not a copy of it.
- Check eligibility and read back copay or preventive coverage.
- Escalate urgent symptoms to the on-call provider and emergencies to 911. See how our after-hours answering service handles nights and weekends.
- Write a summary to the patient record, so nobody calls back to ask what happened.
How to switch from a human answering service
- Request a demo line. A person replies within one US business day with a walkthrough time, the BAA and your EHR's integration status; the demo number follows once telephony is provisioned for you.
- Connect your systems and sign the BAA. Today that means the built-in schedule plus the REST API or CSV export; OpenEMR, athenahealth, eClinicalWorks and Open Dental adapters are sandbox and planned. See integration status.
- Move after-hours calls first. Keep the old service on daytime overflow for a week and compare outcomes.
- Add overflow and lunch calls, then end the per-call contract once the schedule shows the bookings.

Medical answering service questions
What is a medical answering service?
A medical answering service answers a practice's phone when staff cannot, after hours, at lunch or on busy mornings. Traditional services use human operators who take messages; Clinic Answering Service uses HIPAA-compliant AI that books the visit in your EHR, checks insurance and texts the confirmation.
How much does a medical answering service cost?
Human services usually bill per call or per minute; Smith.ai, for example, publishes $300 for 30 calls and $2,100 for 300 calls. Clinic Answering Service is flat: $149, $349 or $699 a month with 300, 1,000 or 3,000 connected minutes, and $0.35 per extra minute.
Is an AI medical answering service HIPAA compliant?
It can be, if the vendor signs a Business Associate Agreement and applies the safeguards in it. Clinic Answering Service signs a BAA on every plan, encrypts calls and records, logs PHI access and never trains public models on patient data. Always ask any vendor for the BAA first.
Is Clinic Answering Service an alternative to Ruby or Smith.ai?
Yes, for practices that want bookings rather than messages. Clinic Answering Service answers instantly, books appointments into real slots (EHR write-back is planned; sandbox today) and runs a 270/271 eligibility check on the call (sandbox until a clearinghouse is contracted). If you want a human voice on every call, a human service may suit you better.
Can patients still reach a person?
Always. Saying "person" or "staff" warm-transfers the call to your front desk or on-call line, and Clinic Answering Service plays a ten-second whisper summary to your staff first so the patient never repeats themselves. Emergencies are told to call 911 immediately.
How long does it take to switch from a human answering service?
Usually one business day of setup plus a week of side-by-side testing. Clinic Answering Service connects your EHR, sets hours and escalation rules, signs the BAA, and then you move after-hours forwarding first and daytime overflow once you trust the results.
Never send a patient to voicemail again.
Ask for a demo and a person replies within one US business day. BAA signed before the first patient call. No long-term contract.
